Please read the following documents prior to beginning the Fast Track Health History questionnaire. The questionnaire will refer to these documents so it is important you read them in their entirety. Please be sure to complete the questionnaire on the same day as your donation and be sure to use your name exactly as it appears on your legal ID. After you complete the Fast Track questionnaire, you will receive a QR code; please bring the QR code with you to your appointment.
