Donating blood is an easy way for minors to have a positive impact on their community. Becoming a volunteer blood donor at an early age helps establish a lifelong commitment to community service and saving lives.

Parents/guardians may complete the consent form online. Click the button below to begin.

About the eConsent:

  • Valid for 1 donation within 42 days of form completion.
  • Enter your minor’s name as it appears on their legal identification – no nicknames or abbreviations.
  • Please note that completing the eConsent does not generate an appointment. Schedule your appointment online at Vitalant.org or contact your school blood drive coordinator.

Paper Parent/Guardian Minor Donor Consent Form

Click the link below to access a printable form which may be signed by a parent/guardian and presented the day of donation by the minor donor.

Eligibility:

  • Must be at least 16 years old.
    • If 16 years old, a signed Vitalant parent/guardian consent form is required each time you donate.
    • 17-year-old donors do not need parental/guardian consent unless their school requires it or they live in these states: Arizona, Colorado, Idaho, Montana, Utah, Washington or Wyoming.
  • Be in good general health.
  • Weigh at least 110 pounds.
  • Meet minimum height and weight requirements for donors 16 through 22 years of age:
    • Male donors: Must be at least 5'0" tall and weigh at least 110 pounds.
    • Female donors: Refer to the chart below for eligibility. ≥ (greater than or equal to).
