At Vitalant, we offer eligible employees a consumer-driven health care benefit plan through Aetna. We contribute to a health fund for employees based on the coverage tier they select. The health fund covers 100 percent of medical expenses up to the coverage tier amount. If your annual expenses exceed the amount provided in the health fund, then there is some cost sharing; however, there is an out-of-pocket maximum. Our health plan provides pharmacy Rx benefits through a tiered co-pay structure. Co-pay amounts vary depending on the type of drug with the lowest co-pay amount being $10 for a 30-day fill for a generic prescription.
The plan pays 100 percent for preventive care services such as physicals, immunizations and health screenings with in-network providers and includes access to tools and resources to help inspire greater overall health and well-being.
Our dental plan is also provided through Aetna. Preventive and diagnostic expenses are covered at 100 percent of the reasonable charge, including cleanings two times per year. Preventive expenses do not apply toward the annual maximum or the deductible. The deductible for all other services is $50 per person or $150 per family. After the deductible, the plan pays 80 percent of the reasonable charge for basic services such as fillings and 50 percent for major restorative services such as bridges.
All benefits (including preventive and diagnostic benefits) are paid up to a maximum of $1,500 per calendar year per covered person. There is a preferred network of dentists; however, network providers are not required for benefits under the plan. Child orthodontia is covered up to 50 percent after the deductible up to a lifetime maximum of $1,000.
Flexible Spending Accounts (FSA)
Health Care FSA
You may choose to have voluntary pre-tax contributions deducted from each paycheck to assist with future reimbursement of eligible medical and dental expenses that are not covered by the health or dental plan. For example, deductible amounts or amounts above plan limits.
Dependent Care FSA
Similarly, you may choose to have voluntary pre-tax contributions deducted from each paycheck to assist with future reimbursement for eligible dependent care expenses, such as day care.
401(k) Retirement Savings Plan
Vitalant employees over the age of 18 are eligible to contribute from zero percent to 100 percent of income up to the IRS annual limit to our 401(k) Plan. Contributions can be made pre-tax or after tax (also known as Roth contributions). You are automatically enrolled into the plan on a pre-tax basis after three months of service. The automatic contribution percentage is 3 percent of eligible compensation and can be adjusted at any time.
We begin matching contributions after three months of service. The matching contribution is an amount equal to 100 percent of the first 3 percent of an employee’s contributions, plus 50 percent of the next 2 percent of contributions. Individual contributions and employer matching contributions are 100 percent vested immediately. Have an existing qualified retirement account with a previous employer? You may opt to transfer or roll over that account into the plan upon eligibility.
Basic Life Insurance and AD&D
Vitalant employees are automatically enrolled in a company-paid basic life insurance plan after 90 days of employment. The death benefit equates to your annual salary rounded up to the next $1,000. The accidental death and dismemberment (AD&D) benefit is an additional year’s salary rounded up to the next $1,000.
Optional Life Insurance and AD&D
You may choose to purchase additional, optional life insurance or AD&D at group rates in the amount of one, two or three times your rounded annual salary. Basic life insurance and AD&D, and optional life and AD&D insurance combined may not exceed $1,000,000.
Dependent Life Insurance
You may choose to purchase life insurance converge for a spouse or domestic partner in the amount of $10,000, $25,000 or $50,000 and/or on a child(ren) in the amount of $5,000, $10,000 or $15,000 per child.
Business Travel Accident Insurance
We provide life insurance coverage with a death benefit of at least $100,000 for death or dismemberment that occurs while traveling on Vitalant business.
Short Term Disability
Vitalant provides income continuation for disability periods after a seven-day (calendar) waiting period and up to 12 weeks. The level of income continuation depends upon an employee’s length of service ranging from 60 to 100 percent.
Long Term Disability
In the event of a medically certified illness or injury that is expected to result in a long-term disability, our insurance provides 60 percent of base wages This policy does not cover pre-existing conditions for which treatment has been received in the three months prior to the plan’s effective date or which begins in the first 12 months of coverage.
Guidance Resources (Employee Assistance Program)
Our employee assistance program through ComPsych is another way we encourage holistic well-being. Through this program, you have access to professional guidance, information, resources and referrals to assist you and your family members with the issues that matter most — from personal concerns to legal and financial concerns. Services are provided at no cost and are completely confidential. Employees and family members are eligible for nine consultations per event per year.
Paid Time Off (PTO) and Holidays
Vitalant employees accrue Paid Time Off (PTO) beginning on the date of hire to cover absences from scheduled work hours, including but not limited to: illness, medical appointments, approved personal business, vacation and any designated floating holidays. Upon hire, you will receive up to 5.24 PTO hours every pay period (17 days per year). This award increases with length of service.
Eligible employees receive six paid holidays per year upon hire: New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day.
Education, Certification and Licensure Reimbursement
We care about ongoing education and training for Vitalant employees. You may apply for reimbursement of 100 percent of tuition and 50 percent of books up to a maximum of $2,500 per calendar year for part-time employees and $5,000 per calendar year for full-time employees.
Similarly, you may apply for reimbursement of 100 percent of professional certification/licensure and special training up to a maximum of $2,500 per calendar year for part-time employees and $5,000 per calendar year for full-time employees. The maximum annual benefit is a combined maximum between the two programs.
Vitalant will reimburse eligible exempt employees for certain expenses incurred within 12 months of transfer or hire if you are relocated with prior approval or at management’s request. To be eligible, your new worksite must be more than 100 miles from your current residence.
